Pathways through Medicaid to Prevention

This toolkit—developed with funding from the Robert Wood Johnson Foundation—illustrates the range of Medicaid and CHIP options that states can use to link clinic to community prevention to address chronic disease, and how to put these strategies in place on the ground. It shows how states have created sustainable financing through Medicaid and CHIP for preventing chronic disease, including childhood obesity, at both the individual and population levels. The toolkit offers states a Roadmap to get started or advance their prevention efforts.
